The Challenge of Manual EOB & ERA Processing

EOBs and ERAs arrive in inconsistent formats from every payer, often as scanned documents. Manually extracting claim-level payment details is slow and prone to error — and errors here ripple straight into your financials.

A biller might handle dozens of EOBs a day, each one formatted differently depending on the payer, with adjustment codes, remark codes, and patient responsibility figures scattered across the page in different layouts. Reading and re-keying that information by hand is exactly the kind of repetitive, detail-heavy task where a single misread digit — a $50 write-off entered as $500 — can throw off a reconciliation days or weeks later.

This challenge is compounded when a single claim spans multiple remittances — a partial payment followed by a later adjustment, for example — since reconciling the full picture requires connecting documents that may arrive weeks apart and reference each other only indirectly.

How Our AI Extracts & Reconciles Claim Data

This pipeline is available today through a controlled pilot engagement, with our RCM domain team validating extracted and reconciled data throughout: it reads EOBs and ERAs regardless of format, extracts claim-level payment details, and reconciles them against expected amounts, flagging discrepancies for review.

Rather than requiring payers to standardize their formats — which will never happen — the pipeline is built to handle the variation directly, recognizing the layout and terminology differences between payers and extracting the same underlying data points consistently. Every extracted figure is then checked against the expected reimbursement, so discrepancies surface immediately instead of during a month-end reconciliation.

The same pipeline also tracks claims across multiple related remittances, linking a partial payment to its later adjustment automatically so your team sees the complete picture for a claim rather than having to piece it together from separate documents received weeks apart.

Adjustment Codes, Write-Offs & Patient Responsibility, Handled Accurately

We correctly interpret CO, PR, and OA adjustment codes, contractual write-offs, and patient responsibility amounts — the details that are easiest to get wrong manually and most costly when missed.

These distinctions matter more than they might seem. A contractual write-off (CO) should never be billed to the patient, while patient responsibility (PR) should be. Getting that distinction wrong — even occasionally — creates compliance exposure and patient billing complaints that take far longer to resolve than the original processing would have taken.

We also flag unusual patterns at the payer level — for example, a payer that has recently started applying an adjustment code more frequently than its historical baseline — since that kind of shift often signals a policy change worth understanding before it affects a larger volume of claims.

Outcomes

This workflow is currently available through Promantra.ai’s controlled pilot program, so we don’t yet have published results specific to EOB and ERA processing — we’d rather tell you that directly than overstate where we are.

A pilot in this area is designed to target faster claim reconciliation, fewer manual corrections downstream, and more accurate financial reporting, with clear baseline and target metrics agreed before work begins, and results published here once verified.
